I would like to have a layout window that shows the History of the DVD (watched by info, for example). I have a simple HTML window which shows Last Watched By and Last Watched Date, but I would like to have a full History list along with the Notes (Date Watched, Watched By, Notes from the Note field in the Watched By pop-up).
Anyone have any ideas on how to create this, or does someone already have one they've created I could see?

Assuming you use the Watched By... (ctrl+W) window to register a viewing event, there are 2 ways to see the viewing history of a profile:
1. Stay at the CTRL+W (menu: DVD -> Watched by...) window, which indeed shows only the last time each possible user has seen the DVD / BluRay. This is much like you mentioned as "a simple HTML window"
2. Use CTRL+H (menu: DVD -> History...), which shows all events, meaning all the times someone viewed the disk AND the window allows you to add a note with the watching event (showing in brackets in the description column), You can also edit already recorded events. It also allows you to export the event history via the clipboard.
To me, it looks like what you want is already provided by the History window. If you want/need something different, please be so kind to explain what is yet missing for your needs. | | | Eric

So LastWatched seems to be the best possible at the moment.
